and Private Appointments. After you confirm your clinic or private appointment, complete the registration form, mail it with your payment

If you are registering late, please confirm availability by calling 516.647.1482 and then fax the form to 516.797.7535. Bring the form and
payment with you to the first appointment.
To all clinic participants: It is important that a registration form is completed and signed by your parents. Note: If you are registering and
paying online, this form is not required. If you have a sibling or team member catch, you must submit a separate form for that participant
signed by their parent. Failure to have a completed form, properly signed along with payment, will prohibit you from participating in a
clinic or appointment.
